Request Incident Report or Public Record

Guided by State of Florida Statute 119 directing that all state, county and municipal agencies must provide access to the public of its records, North Collier Fire is committed to providing an efficient process for the constituents it serves to request records. You may submit your request online via our form provided on this page, or you may call, write or visit us here:

Please be advised that Florida has a broad public records law, and all correspondence to the District via email or electronic web form may be subject to disclosure. Email addresses are public records. If you do not want your email address or the web form released in response to a public records request, do not correspond with the District via email or submit an electronic web form. Instead, contact this office by phone or in writing.

Requirements differ for each type of records request. Please review the requirements below:

Incident Report

For non-medical incidents, you will need the date and location of the incident. Approximate time is also helpful. 

Incident Report with Medical Care

All medical care given by NCFR Firefighter/EMTs and Firefighter/Paramedics is documented in a Patient Care Report (PCR). The data contained in the PCR is considered protected health information under the Health Insurance Portability and Accountability Act of 1996 (HIPAA), the Health Information Technology for Economic and Clinical Health Act (HITECH Act) and the Florida Information Protection act of 2014 (FIPA). The District can only provide medical information for medical care performed by North Collier Fire Control and Rescue District. For reports regarding ambulance transport to hospitals and/or care given by Collier County EMS, please contact Collier County EMS at (239) 252-3740.

The following are general guidelines as to the documents required to process requests for medical records. As each request is unique, additional requirements may apply.

If Requestor is Self:

  • Exact date, time and location of the incident; and
  • A signed HIPAA Authorization for Disclosure of Protected Health Information form. If not appearing in person at North Collier Fire Control and Rescue District so Requestor identity can be verified by a valid driver’s license or government issued photo ID, the patient signature on the form must be notarized.

If Requestor is Acting on Behalf of a Minor:

Parent of Biological/Adopted Minor

  • Exact date, time and location of the incident;
  • Certified copy of minor’s birth certificate; and
  • A signed HIPAA Authorization for Disclosure of Protected Health Information form. If not appearing in person at North Collier Fire Control and Rescue District so Requestor identity can be verified by a valid driver’s license or government issued photo ID, the requestor’s signature on the form must be notarized.

Guardian of Minor

    • Exact date, time and location of the incident;
    • Legal documents validating guardianship; and
    • A signed HIPAA Authorization for Disclosure of Protected Health Information form. If not appearing in person at North Collier Fire Control and Rescue District so Requestor identity can be verified by a valid driver’s license or government issued photo ID, the requestor’s signature on the form must be notarized.

If Requestor is Acting on Behalf of a Deceased Person:

  • Exact date, time and location of the incident;
  • A copy of the death certificate;
  • Documentation validating Requestor is an authorized representative of a deceased patient (acting as an executor, administrator or other person who under applicable law has the authority to act on behalf of the deceased patient or the patient’s estate) or attorney letter representing the estate of the requesting family member; and
  • A signed HIPAA Authorization for Disclosure of Protected Health Information form. If not appearing in person at North Collier Fire Control and Rescue District so Requestor identity can be verified by a valid driver’s license or government issued photo ID, the requestor’s signature on the form must be notarized.
Fire Investigation

Fire investigation reports cannot be released until investigations are finalized by the Fire Inspector. You will need the date, time and exact location of the fire incident.  Confidentiality guidelines apply to arson investigations. 

Property Inspection Reports

North Collier Fire Rescue maintains fire and life safety inspection records for properties within the District. These records relate specifically to compliance with the Florida Fire Prevention Code.

Records Available from North Collier Fire Rescue

Upon request, the District may provide:

  • Open fire code violations;
  • The most recent fire and life safety inspection report on file; and
  • Incident report history for responses to a property involving hazardous materials, if applicable. Incident reports reflect emergency response activity only and do not constitute environmental assessments or environmental clearance determinations.

Records NOT Available from North Collier Fire Rescue

North Collier Fire Rescue does not maintain environmental site assessment records. This includes:

  • Phase I or Phase II Environmental Site Assessments;
  • Contaminated site reports;
  • Petroleum storage tank records;
  • Hazardous waste site documentation; and/or
  • Environmental remediation files.

Requests for environmental records should be directed to the appropriate regulatory agency. For properties located in Collier County, environmental records may be available through the following agencies. Please contact those agencies directly for assistance.

  • Florida Department of Environmental Protection (FDEP), South District Office
  • Collier County Growth Management Division

 

General/Other
You will need to give an explanation of the records you are seeking.  Please note that any confidential information will be redacted from the completed report.
Fees

FEE SCHEDULE

As per Florida Statute 119.07, fees may be charged for the work by Staff to fulfill a public records request. You will be notified prior to fulfillment if fees apply. Fees typically apply to requests that require extensive time of staff to fulfill. In such instances, payment would be required prior to record fulfillment. An invoice will be provided.
