  • We are excited to have you join the Sunshine Fund! Please take a moment to complete this form authorizing our Payroll Team to begin deducting your voluntary contribution each pay period.
  • I request and authorize the North Collier Fire Control and Rescue District to deduct the amount stated above each pay period from my earnings. I understand that the amount deducted is a voluntary contribution to the employee-funded Sunshine Fund, and that I can opt-out at any time.
