Community Care

The North Collier Fire Control and Rescue District’s Community Care program expands emergency medical services beyond the traditional 911 response. Also known as Community Paramedicine or Mobile Integrated Health (MIH), this program allows specially trained Paramedics to provide non-urgent, preventive, and follow-up care within the community—often delivering services where patients live.

Rather than responding only to emergencies, Community Care focuses on connecting individuals to appropriate medical, social, and community resources, helping address underlying needs that may otherwise lead to repeated emergency calls.

How the Program Helps

Community Care initiatives are designed to:

  • Reduce unnecessary 911 calls and emergency department visits;

  • Improve patient outcomes and overall quality of life;

  • Increase access to care for underserved or homebound populations; and

  • Support healthcare system efficiency and responsible use of resources.

This proactive approach allows EMS personnel to deliver care in a way that is both patient-centered and resource-appropriate.

Mobile Integrated Health (MIH)

The District is implementing a Mobile Integrated Health (MIH) program that expands access to care through planned, coordinated responses for patients with lower-acuity medical needs that do not require a traditional emergency response.

MIH services focus on:

  • Planned response teams to assist with non-emergency medical concerns;

  • Connecting patients with appropriate healthcare providers and services; and

  • Follow-up care and referrals to support ongoing health and wellness.

Community Partnerships

As part of its outreach efforts, the District works closely with community organizations to support residents who may need additional assistance beyond emergency medical care.

EMS personnel routinely identify individuals encountered during calls who may benefit from community-based services and make referrals when appropriate. One such partnership includes collaboration with the Naples Senior Center, where referrals are made for patients who may require additional support, resources, or wellness services.

Through Community Care, the District is able to deliver the right care, at the right time, in the right place, helping residents remain healthier while strengthening the overall healthcare system.
